Please note from 1 August 2015 we are unable to take prescription requests over the telephone. We have listened to patient feedback and patients are unable to get through to book appointements, etc.
Requests can be made at any time in person at reception, via your chemist, in writing or by fax (fax number 01706 355379).
Alternatively you can register for online access via the link at the top of this page. New users are required to complete a simple registration prior to using this service.
If you would like to gain access to this service, please come into the surgery and fill in the relevant forms with proof of ID so that an account can be allocated for you.
To assist the receptionist in processing your prescription you will need to give the following details:
- Your full name and address
- The names of all the medication you require
It is important to give the names of all the medication you require to help prevent mistakes occurring.
Please note that all prescriptions would be available 24 hours after being given to the surgery. Prescriptions are kept at reception for four weeks before they are destroyed.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
